When a pensioner dies, his/her survivor needs to submit "Report on Pensioner’s Death" as soon as possible, to your nearest JPS branch office or pension consultation center.
You need to provide the documents to prove his/her death, e.g. a certified extract copy of your Family Registry, medical certification on death issued by a medical doctor.
If you fail to submit the report, the pension due the deceased person may be overpaid and the overpayment must be recovered.
If you live outside of Japan, send the report by post to JPS Headquarters or JPS branch office in charge of latest place of deceased pensioner’s residence in Japan.
