Report Disability Status

To disability pensioners, JPS Headquarters sends a form "report of disability status: medical report" in a specific year we need your report. You receive it in the end of the third month prior to your birth month in the year.
When you receive the report form, you need to have your doctor fill in the medical report section with the diagnosed information on your current disability condition (when you submit the report written in language other than Japanese, attach a translation in Japanese, bearing the name of the translator,) then you need to return the completed report to JPS Headquarters so that we receive it by the end of your birth month. If we do not receive completed report by due date, we cannot confirm your disability condition and it may result in temporary suspension of benefits payments.
